Each character is picked uniformly with crypto.getRandomValues (no modulo bias), at least one from every chosen set, then shuffled; strength ≈ length × log₂(alphabet size) bits.

What do I need to enter?

Length, How many, Uppercase (A–Z), Lowercase (a–z), Numbers (0–9), Symbols (!@#…), Leave out look-alikes (0 O 1 l I). Optional fields can be left blank.

Is the password sent or saved anywhere?

No. It is generated in your browser with crypto.getRandomValues and never leaves your device; it isn't stored in history either.
